Carotid Stenting How to Prepare Should know before it turns into an urgent

Many people do not think about their carotid arteries, until the time a doctor mentions them in a consult room. The risk of having a stroke increases.

What makes carotid artery diseases truly dangerous is how silently it acts. There's no chest tightening or discomfort, and no sign that something is growing within those vessels. In the majority of people suffering from this condition, the initial sign is a brief Ischemic attack- an epoch of weakness, blurred vision or slurred speech. The symptom is gone within a few several hours or minutes. In some cases, it's an entire stroke.

This silence is why screening is important, especially for people over the age of 55 who have an history of smoking or diabetes, hypertension, or an ancestor with a background of having stroke.

How the Process Actually Looks Like

Carotid Stenting is not an open surgery. There isn't any incision inside the neck, there is no general anesthesia, and there is no surgeon physically entering the body to remove plaque.

A vascular specialist will make a small cuttypically at the groin area -and threads a fine catheter through the artery system up to the narrowed portion inside the neck. The blockage is then crossed with care. Small filter devices are generally installed just over the narrowing point to capture any debris that may fall off throughout the operation. Then, a the stent -- which is a metal mesh is inserted into the blood vessel by pressing the plaque against the wall, thereby keeping the blood vessel in place.

From beginning to finish the procedure can take anywhere from one hour to 90 minutes. The patient is asleep, mildly sed and closely monitored throughout the procedure. The majority of people are pleasantly surprised the ease of care it provides.

A Patient Selection Question

Carotid stenting may not be the sole solution for every constricted carotid vein. The severity of stenosis, the history of symptoms, anatomy and general medical condition all decide if surgery, stenting or medical treatment alone is the correct approach.

Patients with severe narrowingusually greater than 70 percent of those who have suffered from neurological symptoms are typically considered for treatment. In those cases that undergo stenting, it is the most preferred option in cases of open surgery that carry a high risk of complications: advanced age coupled with significant cardiac problems previous neck treatment or radiation, unsuitable anatomy for access to surgery, or a relapse following an endarterectomy.

These decisions aren't made quickly or solely by a single doctor. A cardiologist, neurologist and vascular surgeon generally discuss the case in conjunction before a decision is made. This process is in place for a good reasons.

What is it like compared with the Surgical Alternative

Carotid endarterectomy -the open surgical procedure in which plaque is removed physically from the artery is a procedure that has been in use from the 50s. It has a lengthy track of success and is the most popular procedure for the majority of patients who are candidates for surgery.

The CREST trial, that was a study that followed thousands of patients for many years, contrasted endarterectomy and stenting in a direct manner. The most important conclusion was that the long-term outcomes were mostly similar with respect to both. Endarterectomy was associated with a slight increase in heart attack at the date of surgery. Stenting revealed a slightly greater risk of minor strokes during the procedure. The two approaches did not win decisively.

What stenting brought to table did not confer an advantage over surgery -- it was the accessibility to patients with no surgery option that was suitable. This increased the number of patients who could receive treatment, and that isn't an unimportant development.

The recovery process varies significantly. After stenting, the majority of patients return home the next day and return to routine within one week. Endarterectomy usually requires an extended hospital stay as well as an easier return to normal.

Follow-Up and Medication The Part that is Non-Negotiable

Making sure the stent is placed correctly is just the starting point, and not necessarily the conclusion.

After this procedure, the patients are given antiplatelet therapy -- more typically aspirin and Clopidogrel. These drugs reduce the risk of the formation of clots on the stent's surface during the healing process. If you stop them before they are needed or using them in inconsistent ways creates risks of clot formation that this procedure created to avoid. This is among the areas where behavior of patients directly influences the final results.

Regular follow-ups with ultrasound of the carotid is the standard procedure. The ultrasound scans ensure whether the stent is still open, that the flow of blood through the segment being treated is in good condition, and there is no further narrowing that is forming. The issues that are detected in the early stages of imaging can be addressed. The problems that are discovered after a neurologic incident are a different story completely.

What Happens Following and What Isn't

Stenting solves the immediate mechanical issue -an artery that has narrowed dramatically that is precise and reliable. What it doesn't accomplish is to address the underlying biological factors that led to the stenosis first in the first place.

Atherosclerosis is a chronic disease that affects the entire body. Treatment of a single segment of an arterial vein does not stop the process in other areas. Smokers must quit. The blood pressure which has been high for a long time requires constant medical management and dietary adjustments. Cholesterol levels must be monitored and, in the majority of cases the ongoing treatment of statins. Excess weight, physical inactivity and diabetes that is not properly controlled all contribute to vascular damage, regardless of where a stent has been put in place.

The patients who gain lasting benefits from carotid stenting are those who view this procedure in the context of an aid in a larger processnot as a stand-alone solution that allows for an inclination to return to the old ways.

When to have a conversation

If a carotid ultrasound scan or CT scan has revealed significant narrowing of the carotid veins, the ideal time to review your options is before signs begin to manifest, not later.
